Observations: Licensing toured the facility and noted that all carpeted areas were stained and dirty in places of high traffic and use.

Observations: Licensing audited the medication administration records for persons in care. There were a number of entries for prn medications that did not have the result of the prn consistently recorded.

Observations: Licensing audited the weight records of persons in care. One person in care did not have a weight recorded for December 2023.
