MHO Imposed Conditions - Effective October 3, 2025: 1. Nursing professionals were to administer all PRN (as needed) narcotic medication or medications by injection, including insulin. 2. A Licensed Practical Nurse/Registered Nurse, other than the Manager, must work on site for a minimum of 20 hours per week. 3. Any Registered Care Aides (RCAs), who administer medication must complete a Medication Assistant Certification training program approved by Licensing prior to enrollment. 4. The Licensee will submit a written report to Island Health, Community Care Facilities Licensing Program on the 10th of each month for the previous month identifying the following: a. The number of instances where any community health services providers were accessed for treating conditions of persons in care at the facility, specifically for wound care or other medical interventions. b. The number of new admissions. 5. Nutritional Assessments must be completed by a Dietitian or nutritionist for persons in care who are at a nutritional risk and require additional support including but not limited to, diabetic management, and diagnosed food allergies or intolerances. 6. The Licensee will submit a written report to Island Health, Community Care Facilities Licensing Program on the 10th of each month for the previous month identifying the following: a. The number of concerns/complaints received from persons in care, family members or representatives, or other public sources that are related to the care and supervision of persons in care. The monthly report will provide a brief description of the concern/complaint, date received, response from the Licensee and whether the complaint was submitted to Licensing as a reportable incident. b. Status of the Room Check Flow Sheet indicating the documented room checks for each person in care, and notations of what was found and removed. Historic Condition effective date September 8, 2010: • Shelmarie Rest Home will not admit or retain individuals who are not independently mobile in the house or who require the use of mobility aids without prior approval from the Vancouver Island Health Authority, Licensing Program.
