Corrective Actions Required: Ensure a healthy and safe environment is provided at all times. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On September 25 and October 2, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: A minimum of one week duration in between the completion of any construction or maintenance work before children returned to care. Deep cleaning and a ventilation system would ensure all affected areas would provide a safe environment. A five-day advance notice would be provided to parents if any construction or painting work was scheduled. All staff would review the policy, and the records would be maintained on file. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake August 11, 2025. Through the investigation process, Licensing determined strong paint fumes at the facility had been impacted the children in care.

Action/s Required by Licensing: On September 25, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: All staff had ben notified of the proper exit and transition protocols, and a notice would be posted on the wall in Cedar Room stating the exit door was not allowed for transitioning children from another rooms. The corrective action plan was reviewed and accepted by Licensing.
