Corrective Actions Required: Ensure that the community care facility is operated in a manner that will promote the health, safety and dignity of person in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 24, 2026 and August 6 and 13, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a person in care had an increase in monitoring and was redirected as needed, the Island Health Person Centered Support Team had been consulted on June 24, 2026, for behavior plan review of a person in care, the Geriatric Psychiatrist had assessed a person in care on July 17, 2026 and Island Health Dementia Video Series education had been started for all staff and will be completed by September 30th. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake June 9, 2026. Through the investigation process, Licensing determined that a person in care was aggressive toward another person in care on multiple occasions causing injury.

Description: Subject to subsection (3), if a person in care is involved in a reportable incident, the licensee must immediately notify a medical health officer, in the form and in the manner required by the medical health officer
Corrective Actions Required: Ensure that if a person in care is involved in a reportable incident, the licensee must immediately notify a medical health officer, in the form and in the manner required by the medical health officer. Action/s Required by Licensing: On July 24, 2026 and August 6 and 13, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the nursing team was to complete education related to Reportable Incidents to Licensing, and this was to be completed by August 14, 2026. The corrective action plan was reviewed and accepted by Licensing.
