Observations: Licensing conducted entanglement tests on all slides in the designated play spaces for the GCC (Under 36 Months) and GCC (30 Months to School Age) programs. Licensing observed that the following three slides failed the entanglement tests. - In the GCC (30 Months + ) program, the top of the enclosed blue slide had an entanglement hazard on both the left and right sides where a repair had been recently completed to the slide bed entrance. - In the GCC (Under 36 Months) program area, the yellow wavy slide structure had entanglement hazards at the top left and right slides where the fasteners had popped up above the slide bed surface. Additionally, the Little Tykes Castle slide had entanglement hazards at the top left and right sides of the slide where the slide bed adjoined the structure. - These three entanglement hazards represent an immediate risk to the health and safety of persons in care and must be corrected immediately. - Manager provided a verbal corrective action plan that all three slides would be immediately closed. The facility is closing at the end of the business day for a summer break and will be reopening the first week of September 2023. During this time, maintenance will be conducted. - Manager stated that the equipment would not be used by children until Licensing had returned to reassess for compliance. - Licensing accepted this Corrective Action Plan, and observed that facility staff closed access to these three slides during the inspection.
