Description: The licensee must review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

Action/s Required by Licensee: Licensee to submit a corrective plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 6, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a spreadsheet had been created to track which children had a care plan and the date in which it needed to be reviewed. Additionally, reminders were set for care plan review meetings and all parents who needed to review care plans would be contacted a month prior to the review date. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake January 30, 2026. Through the investigation process, Licensing determined that a care plan dated September 24, 2024, had been developed for a child known to be impulsive and to use inappropriate language; however, the care plan had not been reviewed annually. The absence of an annual review may have limited the facility's ability to update mitigation strategies for known behaviour risks.
