Corrective Actions Taken By Licensee: Licensing observed an email dated October 25, 2022 that included a photograph of a nap room. The photograph indicated that a play pen had been removed and a bed guard rail had been added to a low profile bed.

Observations: Licensing reviewed three staff files and observed one file was missing a valid first aid certificate. Licensing observed the Licensee state that at times the staff member would be caring for children without another staff present.

Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
