Observations: Licensing observed a baby monitor in a person in care's room and a surveillance camera in the kitchen area. The surveillance camera displayed the picture of a person in care's bedroom. There was no signage in a prominent place notifying of surveillance usage.

Observations: Licensing audited two care plans for persons in care and one care plan did not have the updated information related to the use of a baby monitor in the bedroom to monitor falls. Staff stated the implementation of the baby monitor was in April 2025. Staff stated the person in care was now using a wheelchair which was not noted in the care plan.
