Action/s Required by Licensing: On May 20, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the Manager reviewed the facility's policy on complaints and concerns, and submitted written answers regarding actions and responsibilities required as a Manager, when complaints were received. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake April 28, 2026. Through the investigation process, Licensing determined that the licensee had not kept a record respecting complaints and concerns expressed to the licensee, and the responses to them.
