Description: For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

Corrective Actions Taken By Licensee: On January 17, 2022 Licensing received a photograph showing that the opening on the elevated equipment had been repaired and now measured 3.4 inches.
Actions Required By Licensing: No further action required; this contravention has been addressed.
