Observations: Licensing observed the laundry facility. The Manager stated that one person in care used the laundry facilities, and there was no slip resistant material.

Observations: Licensing audited two person in care's Medication Administration Records. Two person in care's medications, scheduled for June 16, 2025, on three administration times, had not been signed and/or coded for by staff.
