Observations: The Point of Care testing/portal system has not been set up for the facility as required by the Ministry of Health as of September 8, 2021. Licensee Contact stated that all staff and persons in care have been vaccinated.

Observations: Upon entry, Licensing Officer was not actively screened. Greeter/screener was not wearing eye protection and did not ask screening questions. Licensee Contact stated that active screening has not occurred with persons who enter the building.
