Corrective Actions Required: Ensure a healthy and safe environment is provided at all times. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between March 3 and March 6, 2026, Licensee Contact submitted a corrective action plan to Licensing, The Corrective Action Plan included: A pest control service had been contacted and completed an inspection on February 24, 2026, and the service would conduct a six-months follow up inspection service. Two door sweeps were installed on the emergency exits in Maple room to prevent rodents from entering the facility from outdoors. Ten Snap traps were placed locations throughout the facility that are inaccessible to children. A Pest Monitoring Log and an Integrated Pest Management Plan had been implemented, management would review the documents daily to ensure consistent monitoring. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake February 14, 2026. Through the investigation process, Licensing determined that staff appeared to sanitize toys and equipment at an appropriate frequency. However, staff were aware of the mouse droppings, and Licensing observed mouse droppings in an area accessible to children.
