Description: A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Corrective Actions Required: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care, specifically regarding suitability of staff members and following children's care plans. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved in this incident had been terminated on December 19, 2024. New employees were provided with all facility policies and direct observation of new staff related to behavioral guidance would be performed by management. Management would review all children's care plans with staff to ensure care plan steps were followed. The corrective action plan was reviewed and accepted by Licensing.

Observations: Through the investigation process, Licensing determined that a staff did not demonstrate the skills and abilities to carry out the duties as assigned. Specifically, that a staff had not used age appropriate behavioral guidance with a child in care and had not followed the child's care plan.
3030 - CCLR 51(1)(a)
Description: A licensee must ensure that behavioural guidance is appropriate to the age and development of the child who is receiving the guidance,
Corrective Actions Required: Ensure all staff follow the Behavioral Guidance Policy at the facility. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: All staff would review the Behavioral Guidance Policy and Child Abuse Policy by January 27, 2025, and on January 30, 2025, all staff would attend a mandatory staff meeting to discuss guidance policies. The corrective action plan was reviewed and accepted by Licensing.

Observations: Through the investigation process, Licensing determined that a staff did not follow the Behavioral Guidance Policy at the facility.

Description: A licensee must ensure that a child is not, while under the care or supervision of the licensee, subjected to any of the following types of abuse or neglect: physical abuse
Corrective Actions Required: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care, specifically regarding staff not physically abusing a child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved in this incident had been terminated on December 19, 2024. 3070 - CCLR 55(1)(b)
Description: A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child is involved in, or may have been involved in, a reportable incident described in Schedule H.
Corrective Actions Required: Ensure that parents of a child who has been or may have been involved in a reportable incident are notified immediately. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 22, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Parents or emergency contacts would be notified immediately by phone or Kindertales app if their child was involved in a reportable incident. The corrective action plan was reviewed and accepted by Licensing.
