Observations: Licensing observed excess spray foam used to correct an entanglement hazard on the slide was accessible to children and could easily be picked off. Manufactures instructions stated hazardous to children when ingested.
3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment)
