3000 - SPS DOLSOP (Depth)
Description: Depth guidelines for loose fill material (if protective surfacing used) :
minimum 15 cm (6”) for 1.5 metre (5’) equipment height
minimum 30 cm (12”) for 2.3 metre (7’) equipment height
(Consider surfacing table, Triax, etc.)
Corrective Actions Required: Ensure all elevated equipment with a five foot fall height has a minimum of 6 inches of depth surfacing.

Observations: Licensing measured depth surfacing under two pieces of elevated equipment with a five feet fall height. Pea gravel measured between 1 and 2 inches under and around the five foot metal dome and in some areas pea gravel measured 2 inches under the monkey bars on the composite climber with the triple slide.
3010 - CCLR 19(1)(a)
Description: A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: a criminal record check for the person

Corrective Actions Taken By Licensee: Licensing received valid staff CRCs by email on February 2, February 14 and February 22, 2023. All three valid CRCs now received.
Actions Required By Licensing: No further action required; this contravention has been addressed.

Corrective Actions Taken By Licensee: Licensing received two missing character reference letters by email on February 10, 2023 for one employee and one missing character reference letter by email for a second employee on February 16, 2023.
