The corrective action plan included: Staff were to conduct head counts, and the manager would cross check to ensure maximum group size was not exceeded. The corrective action plan was reviewed and accepted by Licensing.

Corrective Actions Taken By Licensee: On November 28, 2025, Licensing observed there were no more than 12 children in the Group Child Care (Under 36 Months). The attendance was as follows; six children; four children; three children and two children in each room and staff were completing head counts and the manager was cross checking to ensure maximum capacity was not exceeded.

Observations: Regarding intake July 22, 2025. Through the investigation process, Licensing determined that the max group size as per Schedule E, which states the max group size is 12 was exceeded when the Group Child Care (Under 36 Months) programs combined and had a total of 22 children.

Description: Without limiting subsection (1) a licensee must ensure that children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E, (except FCC/IHMACC/REC; child qualified if in age range or within one month (except GCC School Age)).

Observations: Regarding intake July 22, 2025. Through the investigation process, Licensing determined there was one staff ECE (replacing absent ITE) in the Group Child Care (Under 36 Months) program with six children, when an additional ECE was required in a group of 5-8 children. On November 28, 2025, Licensing observed at 8:22 a.m. Licensing observed two staff (ITE, ECEA) with five children in the Group Child Care (Under 36 Months) program. This did not meet the Schedule E, when ITE was required for 1 to 4 children and an ITE and ECE was required for 5-8 children.

Observations: Regarding intake July 22, 2025. Through the investigation process, Licensing determined that on August 22, 2025, the staff to child ratios were not maintained when the Group Child Care (Under 36 Months) room had three staff (ECE, ECE, ECEA) with ten children. The Infant and Toddler Educator was scheduled off on August 22, 2025. This did not meet Schedule E when one Infant and Toddler Educator, one other Educator and one Assistant was required for 10 children in care. In addition, employee's lunch breaks were being covered by a lesser qualified Educator. On November 28, 2025, at 7:57 a.m. Licensing observed one staff (ECEA) in the Group Child Care (Under 36 Months) program with one child. The staff stated they had started their shift early in this room and the parent had completed the transfer of care in this room instead of the assigned one. The staff stated the ITE was coming at 8:00 a.m. from the designated Group Child Care (Under 36 Months) drop off room. The ECEA went to the designated room with the child. Licensing observed the staff break scheduled that indicated qualified staff were to replace staff lunch breaks.

The corrective action plan included: Daily attendance would be completed by the Educators and verified by the manager. Staff would sign in children when children arrived and departed from one classroom to another. The corrective action plan was reviewed and accepted by Licensing. On November 28, 2025, Licensing observed a record was not being kept indicating the arrival and departure times to and from rooms.

Through the investigation process, Licensing determined that the Group Child Care (Under 36 Months) programs combined until 8:00 a.m. and the children did not sign in to attendance records upon arrival and they did not sign out of attendance records until after 8:00 a.m. The children were signed in to attendance by their parents into their respective room that they were registered in.
