Observations: Licensing audited three employee files. All did not include a record that the performance of each employee has been reviewed regularly. The Human Resources Manager indicated that performance reviews had not been completed.

Description: A licensee must keep, for each person in care, a record showing the following information: name, sex, date of birth, medical insurance plan number and immunization status
