3010 - RCR 42(1)(a)
Description: A licensee must ensure that, at all times, the employees on duty are sufficient in numbers, training and experience, and organized in an appropriate staffing pattern, to meet the needs of the persons in care,
Corrective Actions Required: Ensure that the employees on duty are sufficient in numbers, training, and experience, and organized in an appropriate staffing pattern to mee the needs of the persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On August 6, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: care staff and nursing staff received education regarding the importance of documentation regarding baths including refused or missed bath on August 6 and 13, 2026, the staff workload was adjusted following any missed baths, two bathing lines and float lines had been filled as of July 17, 2026, and ongoing attendance management would occur with staff who repeatedly did not attend shifts. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake July 14, 2026. Through the investigation process, Licensing determined that the employees on duty were not sufficient in numbers, training, and experience, and organized in an appropriate staffing pattern to meet the needs of the persons in care.
3030 - RCR 52(1)(a)(iii)
Description: A licensee must ensure that a person in care is not, while under the care or supervision of the licensee subjected to any of the following types of abuse or neglect, as described in section 1 of Schedule D: neglect
Corrective Actions Required: Ensure that a person in care is not subjected to neglect, while under the care or supervision of the Licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On August 6, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: all care plans were reviewed to ensure bathing date was consistent with electronic bathing record system, care staff and nursing staff received education regarding the importance of documentation regarding baths including refused or missed bath on August 6 and 13, 2026, the staff workload was adjusted following any missed baths, two bathing lines and float lines had been filled as of July 17, 2026, and ongoing attendance management would occur with staff who repeatedly did not attend shifts. The corrective action plan was reviewed and accepted by Licensing.
