3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment) Use Entanglement test device

Observations: Licensing used the entanglement device three times on each side of the slide. Test failed each of the six times. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. A plan for corrective action must be submitted to Licensing by June 15th, 2022, by 9 a.m. Full compliance will not be be achieved until the plan has been implemented and completed.
3000 - SPS DOLSOP (Entrapment)
Description: For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

Observations: Licensing observed spaces between the wooden slats on railing and between stairs of climber were between 3.5 inches and 9 inches. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. A plan for corrective action must be submitted to Licensing by June 15th, 2022, by 9 a.m. Full compliance will not be be achieved until the plan has been implemented and completed.
3000 - SPS DOLSOP (Equipment height)
Description: Height of equipment – Highest play surface as reported by Licensee or observed by LO: for toddlers (12 to 36 months) no more than .9 metres (3 feet) for preschool children (30 months to school age, no more than 1.5 metres (5 feet) for school-age children (attending school) no more than 2.3 metres (7 feet)
