Corrective Actions Required: Ensure to operate the community care facility in a manner that will promote the health, safety & dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 14, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: a staff member was not longer employed effective immediately. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake March 31, 2026. Through the investigation process, Licensing determined that a staff member did not demonstrate the skills to carry out the duties assigned when they physically guided children's behaviours, "pulled" a child out of the bathroom and instructed the child to stand by the wall as per behavioural guidance. They were frequently raising their voice during transitions and using a firm tone when expressing they were "angry" or directing children to complete tasks, including stating, "I have told you to do this"

Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention.
