3000 - SPS DOLSOP (Entrapment)
Description: For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

Corrective Actions Taken By Licensee: Ensure that opening on elevated equipment measure less thank 3.5 inches.
Actions Required By Licensing: Follow Up Inspection to be conducted.
To Be Corrected By: January 11, 2023
Observations: Licensing used the head and torso probes and detected an entrapment on elevated equipment that measured 4.7 feet in height.
3010 - CCLR 22(2)(c)
Description: A licensee must ensure that each employee practices implementing the emergency plan at least once each year.
Corrective Actions Required: Ensure an emergency plan is practiced once a year.
