Corrective Actions Required: Ensure that the facility is operated in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On February 19, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a pest control company was scheduled on site bi-weekly for three months with monthly communication regarding activity in traps with actions to reduce rodent activity, rodent traps were placed in person in care suites where rodents had been noticed, a memo was sent out on February 4, 2026 to all departments to document sightings of rodents and maintenance will document any reported sightings with dates, locations and actions taken. The corrective action plan was reviewed and accepted by Licensing.
