Observations: Licensing toured the facility and observed the couch in the living room to be tattered.

Observations: Licensing audited the medication administration record and observed that PRN results had not been documented for one person in care on April 1 and April 12, 2023. Licensing observed that two PRN medication blister packs did not have an expiry date included on the labels.
