Observations: This item has been brought forward from Routine Inspection dated February 24, 2024. On this date, Licensing observed physical damage to a person in care's door and wall.
3070 - RCR 81(3)(c)(ii)
Description: A care plan must include all of the following: a nutrition plan that specifies the nutrition to be provided to the person in care, including the requirements of any therapeutic diets

Observations: Licensing reviewed the weight charts of two persons in care for the last five months. One person in care did not have a weight recorded for one month and one person in care had two months where their weight was not recorded.

Description: A licensee, other than a licensee who provides a type of care described as Hospice, must provide persons in care, without charge, with an ongoing planned program of physical, social and recreational activities suitable to the needs of the persons in care
