Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 18 and August 1, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a staff was to participate in a care plan review with leadership, review and sign off on "Managing the Potential or Actual Presence of Weapons or Prohibited Items" and Long-term Care Licensing course through Learning Hub by August 15, 2025 and staff would have an education session on correct documentation procedures in the Electronic Health Record (EHR) by September 30, 2025. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake May 16, 2025. Through the investigation process, Licensing determined that staff did not have the skills and abilities necessary to carry out the duties assigned.
