Observations: Licensing observed large scissors, rusty metal tools, broken play equipment, large garbage bins filled with plastic bags and kitty litter, a large wooden beam balanced against an outdoor wall and a deep bucket filled with 2 feet of standing water in the outdoor play area. Exterior fence doors and trampoline did not have the locking latches used to secure them in the outdoor play area. A large metal locker cabinet had been added to the entry way of the facility and was not bolted to the wall in the indoor play area. Licensee indicated the bucket of water would be emptied today before children played in the outdoor play area.
3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment)
Use Entanglement test device
Corrective Actions Required:
Corrective Actions Taken By Licensee: Licensing used the entanglement device on an outdoor composite slide that measured 3.1 feet and did not detect an entanglement.
Actions Required By Licensing: No further action required; this contravention has been addressed.

3000 - SPS DOLSOP (Pools)
Description: Use of pools/trampolines/other non-typical equipment has careful and detailed consideration on a case-by-case basis (Considerations include: whether equipment meets needs of children what additional types of risk are involved what necessary steps will be needed to ensure adequate active supervision & how will staff ensure the equipment is used & maintained properly)
Corrective Actions Required: Ensure a plan is submitted to Licensing for the use of the mini trampoline.

Observations: Licensing observed a mini trampoline in the outdoor area. Licensee stated the children use it but there has been no plan submitted to Licensing for review.
3020 - CCLR 56(1)(c)
Description: A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment]
Corrective Actions Required: Licensee to ensure implementation of fire drill systems are recorded.
