Observations: Licensing observed the fire extinguisher was due for servicing in June 2025.

Observations: Licensing observed a mattress added to a pack and play. Manufacturer instruction state not to add any mattress. Licensing also observed outdoor check list was last completed July 4, 2025. Licensee stated she's completed the check each morning but hadn't recorded them.
