Corrective Actions Taken By Licensee: Licensing observed the Licensee provided individual hand drying towels for each child. The name of the child was written on the towel and hung separately apart on the washroom wall.

3070 - CCLR 56(1)(f)
Description: A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Corrective Actions Required: Ensure a minor incident, illness and unexpended events involving children log is kept.

Observations: Licensing observed the Licensee has not keeping a minor incident log. Licensee informed the facility had been providing care for less than 30 days since issuance of licence.
3070 - CCLR 57(2)(a)
Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Corrective Actions Required: Ensure for each child record a medical insurance plan number and immunization status is obtained.
Corrective Actions Taken By Licensee: Licensing observed there was one of two medical insurance numbers obtained (one remains outstanding) and one immunization record obtained, one immunization record left outstanding and two immunization records were not obtained as the chidlren were no longer attending the program. This contravention remains outstanding until all items are completed.
Actions Required By Licensing: Follow Up Inspection to be conducted.

Observations: Licensing observed six children's records, two were missing medical insurance plan number and four were missing immunization status.

Description: A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

Corrective Actions Taken By Licensee: Licensing observed the child was no longer attending; the Licensee informed the physician name and number was not obtained form the parent.
