Observations: Licensing toured the facility and observed damage to the walls, doors, and stairwell throughout the facility.

Observations: Licensing toured the facility and observed one bedroom to have cigarette butts scattered all over, one bedroom had a strong odour coming from it, and two bathrooms to be notably in a uncleanly state.
