Description: A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Corrective Actions Required: Ensure to operate the community care facility in a manner that will promote the health, safety & dignity of persons in care, specifically, policies. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 2, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: a formal parent notification process would be implemented to ensure families were informed in writing before any practicum student or similar individual began placement. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake April 29, 2026. Through the investigation process, Licensing determined there was no evidence indicating that parents were informed, in advance, that a practicum student would be observing a child, as outlined in the "Teachers" policy in the parent handbook.

Description: A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Corrective Actions Required: Ensure to notify the Medical Health Officer within 24 hours after a child is involved in or may have been involved in a reportable incident described in Schedule H while under the care or supervision of the Licensee. Action/s Required by Licensee: Licensee Contact to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 2, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: Staff would reinforce reportable incident procedures and staff would receive ongoing training on reporting, communication, and documentation. Tracking systems would be kept to ensure accurate monitoring and record keeping of incidents. The corrective action plan was reviewed and accepted by Licensing.
