Corrective Actions Required: Ensure the community care facility is operated in a manner that promotes the health, safety and dignity of persons in care, specifically that policies for termination of care and refunds are followed. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 14 , 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the Licensee refunded the deposit for providing less than two weeks notice of termination of care and made changes to their polices for termination of care and refunds effective September 15, 2026. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake June 4, 2026. Through the investigation process, Licensing determined that the termination of care and refund policy were not followed when insufficient notice was provided.
