Observations: Licensing reviewed communication book for classroom 4. Licensing observed that an incident occurred on May 9, 2022 where a child fell while in the bathroom and sustained an injury. On May 10, 2022, it was reported to the facility that the cut was glued by a medical practitioner. Incident was not reported to Licensing.
