Corrective Actions Required: Ensure that care plans for persons in care are reviewed and modified when there is a substantial change in their care needs. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On April 15, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included a person in care's care plan was revised to include refusal of oral care, documentation requirements of staff when care was refused, and staff were to inform the nurse on duty. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake dated February 18, 2025. Through the investigation process, Licensing determined that the care plan for a person in care had not been updated to include refusals of oral care and the documentation required by the staff.
