Observations: Licensing observed large wooden blocks in the fall zone of a small tire swing.

Description: For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

Observations: Licensing observed a four foot indoor climber had cutouts on the top platform that measured 4.5 inches.
