Description: A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Corrective Actions Required: Ensure that a current log is kept to record all minor accidents, illnesses and unexpected events involving children. Actions/Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between March 6 and March 11, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: staff would use an incident chart [tracking tool] to indicate each day when a minor incident had occurred. The chart would be used to remind staff to complete documentation and to ensure that the information was documented in the facility 'App'. The Manager and another staff would review the minor incidents that were tracked along with the documentation of minor incidents. Staff would additionally sign off on the corrective action plan to acknowledge the new procedure to track and document minor incidents. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake January 19, 2026. Through the investigation process, Licensing determined that staff had observed [child A] push, kick, and hit other children. Staff verbally informed parents of the minor incidents [child A] was involved in. While the incidents did not meet the criteria for submitting a reportable incident, the Licensee was required to maintain records of all minor accidents, illnesses and unexpected events that involved [child A]. Licensing did not observe any associated documentation at the facility that [child A] had pushed, kicked, and hit [unexpected events].
