Corrective Actions Required: Ensure to operate the community care facility in a manner that will promote the health, safety and dignity of persons in care, specifically, suitability of the Licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On May 15, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: a "Breach of Confidentiality Prevention" policy was developed to ensure the secure and respectful handling of all information. Staff were required to use approved communication tools and speak privately when needed. The policy would be reviewed with all staff members and would be reinforced regularly. In the event of a potential breach, they would report immediately to management, Licensing and families. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake April 21, 2026. Through the investigation process, Licensing determined that the Licensee Contact, breached confidentiality when they spoke with a child's parent in a public setting about concerns related to the child. The Licensee Contact confirmed other members of the public witnessed and heard the conversation held with the parent.
