Corrective Actions Taken By Licensee: Licensing observed that all children enrolled had medical service numbers.
Actions Required By Licensing: No further action required; this contravention has been addressed.

Observations: Licensing observed during the course of the inspection that two children's files did not have a photograph of the child.
