Observations: Licensing toured facility and observed that a recliner in the living room had ripped seams and the stuffing was coming out.
3000 - RCR 35(2)(a)
Description: A licensee must ensure that laundry facilities if used by persons in care, have a slip resistant floor surface
Corrective Actions Required: Ensure the laundry facilities have non slip floor surface.

Observations: Licensing observed that the laundry facilities located downstairs did not have not slip floor surface.
3070 - RCR 49(2)
Description: A licensee must record the height and weight of each person in care on admission.

Observations: Licensing reviewed three person in care's records, and observed that one care plan did not have the height of the person in care documented.
