Observations: Licensing used the entanglement testing tools and observed 3 x entanglement hazards detected on both sides of the green slide that had a fall height of 4'. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. The Licensee stated at the time of inspection that children would not be permitted access to the slide structure and would be taken on walks or bike rides until the entanglement hazard was repaired. Licensee to submit evidence that the entanglement hazard was corrected prior to allowing children use of the slide. Full compliance will not be achieved until the plan has been implemented and completed.

Description: A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure
