3030 - CCLR 39(1)
Description: A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Corrective Actions Required: Ensure staff continuously supervise children in the before and after school programs. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On December 19, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: A Transfer of Care policy has been created and will be added to the Parent Handbook which will be sent to all families on January 5, 2026. This policy includes: Children in kindergarten will be walked to their classroom line-up by a child care staff member. The child care staff will stay with the children until the teacher opens the classroom door. This will continue until the routine is established and the children are able to walk to their class line-up at the same time as the older children. Some children may require this transition support for the entire school year. Determining when this support ends is child dependent. Staff are to write down the time children leave the program on the attendance sheet. This corrective action plan was reviewed and originally accepted by Licensing. Licensee changed and updated the corrective action plan to include all staff would review the supervision changes at the
February 17, 2026 staff meeting. Licensing reviewed and accepted the updated corrective action plan on January 12, 2026.

Observations: Regarding intake dated December 8, 2025. Through the investigation process, Licensing determined two Responsible Adults working in the before school program had not continuously supervised a child in the morning program as the child was found alone and crying by another parent off the school grounds.
3070 - CCLR 55(2)(a)
Description: A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Corrective Actions Required: Ensure to notify Licensing within 24 hours after a child was involved in, or may have been involved in, a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On December 19, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Licensee Contact will review licensing requirements for reportable incidents with all staff on January 16, 2026, at a Professional Development Day. The Licensee Contact will follow up with staff not in attendance by January 23, 2026. This corrective action plan was reviewed and originally accepted by Licensing. Licensee changed and updated the corrective action plan to include all staff would review Reportable Incident protocols at the February 17, 2026, staff meeting. Licensing reviewed and accepted the updated corrective action plan on January 12, 2026.

Observations: Regarding intake December 8, 2025. Through the investigation process, Licensing determined an incident of missing and wandering occurred on December 5, 2025. This incident was not reported to licensing within 24 hours.
3070 - CCLR 57(2)(c)
Description: A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure
Corrective Actions Required: Ensure a record is kept for all children showing the time of arrival and the time of departure. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On December 19, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included new attendance records had been created to show time of arrival and time of departure in both the morning and afternoon school age programs. This corrective action plan was reviewed and originally accepted by Licensing. Licensee changed and updated the corrective action plan, to include all staff would review the requirements for attendance records at the February 17, 2026, staff meeting. Licensing reviewed and accepted the updated corrective action plan on January 12, 2026.
