Observations: Licensing toured the facility and observed a cabinet that had signage which stated it must be kept locked, which was unlocked and had chemicals accessible to persons in care.

Observations: Licensing toured the facility and observed damaged walls and doorframes in the hallway, in addition to electrical outlets that appeared to be missing fixtures and covers.

Observations: Licensing audited the medication administration record (MAR) and observed that one person in care did not have a medication signed or coded for by staff on October 4, 2025, and another person in care did not have four medications signed or coded for by staff on October 14, 2025.
