Observations: Licensing observed that the criminal record check for Licensee had expired on June 29, 2020. Licensee stated that they have begun the process of applying for a criminal record check.

Observations: Licensing reviewed five of thirty-two children's records. Five children's records were missing signed copy of repayment agreement. Licensee stated that they are in the process of providing updated parent packages and signed repayment agreements.
