Description: A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Corrective Actions Required: Ensure that a community care facility operates in a manner that promotes the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 3 and 6, 2025 Licensee submitted a corrective action plan to Licensing. The corrective action plan included: new guidance strategies for all staff were created to support children's development, reportable incidents will be submitted to Licensing within 24 hours, the Licensee Contact will review completion of the minor incident log weekly and a new proposed Manager will be appointed to the facility for 2025. The corrective action plan was reviewed and accepted by Licensing. On March 14, 2025, Licensing conducted a Follow Inspection to Complaint Inspection and confirmed that all staff on site had received training related to guidance practices, the Licensee contact was not on site at time of inspection.

Observations: Regarding intake dated December 16, 2024, Through the investigation process, Licensing determined that the Licensee Contact had extended absences from the site, and was not accessible to staff leaving the facility with no Licensee Contact or Manager for extended periods of time and not demonstrated the skills and abilities to carry out the duties assigned.
3010 - CCLR 37(3)(b)
Description: Despite the employee qualifications required by Column 4 of the table in section 1 of Schedule E [group sizes and employee to children ratios], a licensee must replace an absent employee described in Column 1 of the following table who is absent (b) for 30 consecutive days or more with a person described in Column 3 opposite that employee and the row in Column 2 labelled as '= 30 days' (see table listed under Absent employees in CCLR)
Corrective Actions Required: Ensure that a Manager who is absent for more than 30 consecutive days is replaced with a person qualified to be a manager. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 3 and 6, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: On February 27, 2025, documentation for a proposed new Manager was received by Licensing. The corrective action plan was reviewed and accepted by Licensing. On March 14, 2025, Licensing conducted a Follow Inspection to Complaint Inspection and confirmed that a proposed Manager was being trained and had signed up to the Licensing Orientation session on March 20, 2025.

Observations: Regarding intake dated December 16, 2024. Through the investigation process, Licensing determined that there had been no Manager working at this facility for two months and there had been no notification that the Manager had ended employment.

Description: A licensee must ensure that behavioural guidance is appropriate to the age and development of the child who is receiving the guidance,
Corrective Actions Required: Ensure that behavioral guidance is appropriate for the age and development of the child receiving the guidance. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 3 and 6 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: New guidance practices had been created to support children's development with positive supports, children would not be subject to confinement or restraint, staff would receive continuous education and training regarding the guidance practices. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake dated December 16, 2024. Through the investigation process, Licensing determined that the behvaiour guidance strategies used by staff were not appropriate to the age and development of children in care, specifically a staff member not providing explanation of expected behaviours as per the facility policies and several staff applying restraints as part of behvaiour guidance practices.
3030 - CCLR 52(1)(b)
Description: A licensee must ensure that a child, while under the care or supervision of the licensee, is not subjected to confinement or physical restraint by an employee, except as authorized in a child care plan if the plan includes instructions respecting behavioural guidance.
Corrective Actions Required: Ensure that a child is not subjected to confinement or physical restraint. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 3 and 6, 2025 Licensee submitted a corrective action plan to Licensing. The corrective action plan included: New guidance had been implemented at the facility that indicated that children would not be subjected to confinement or restraints, this was to be continuously reviewed with all staff. The corrective action plan was reviewed and accepted by Licensing. On March 14, 2025 Licensing conducted a Follow Inspection to Complaint inspection and received confirmation that staff had received training that included actions not permitted such as restraints.

3070 - CCLR 55(2)(a)

Action/s Required by Licensing: On March 3 and 6, 2025 Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Reportable incidents would and must be reported to Licensing within 24 hours. The corrective action plan was reviewed and accepted by Licensing. On March 14, 2025, Licensing conducted a Follow Inspection to Complaint Inspection and confirmed that all staff on site had received training related to incident reporting within 24 hours.

Action/s Required by Licensing: On March 3 and 6, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: staff will receive education and training related to keeping a minor incident logbook, this will be reviewed by the Licensee contact on a weekly basis. On March 14, 2025, Licensing conducted a Follow Inspection to Complaint Inspection and confirmed that a minor incident log was now implement and kept current.
