Observations: Licensing inspected the rooms of persons in care. The flooring under each of their beds has torn from movement caused by the hospital type bed.

Corrective Actions Taken By Licensee: Licensing received copies of weight records for persons in care. They were recorded on a paper document that was not able to be located during the Routine Inspection.
