Observations: Licensing observed multiple dents on the bottom of the front door, black scuff on the bottom of the wall in different areas, the floor lifting on the laminate and a hole on the wall in the back of the facility.
3000 - RCR 35(1)(c)
Description: A licensee must provide the following appropriately furnished and equipped areas: secure, safe and adequate storage areas for cleaning agents, chemical products and other hazardous materials

Observations: Licensing observed bath tub cleaner next to the toilet in the bathtub room. Bath tub room door was unlocked and accessible to persons in care.
3020 - RCR 74(1)(b)(i)
Description: Subject to subsection (2), a licensee may restrain a person in care if there is agreement to the use of a restraint given in writing by both the person in care, the parent or representative of the person in care or the relative who is closest to and actively involved in the life of the person in care,

Observations: Licensing audited three care plan. One of the care plan had a written restraint agreement form that was not signed by the representative/parent.
