Observations: Licensing observed four of four staff files. Three files were missing valid criminal record checks. Licensing observed evidence that the CRC applications have been submitted and are in progress.

Corrective Actions Taken By Licensee: Licensing received copies via email on September 05, 2023 of photographs taken of the immunization record written, signed and dated by the employee.
