Observations: Licensing observed the bathroom for persons in care was locked as an environmental restraint. The Manager stated this bathroom was locked at all times for the health and safety of persons in care. There was not an agreement for the use of the restraint given in writing by the person in care, the parent or representative of the person in care.

Description: A licensee must keep a copy of each policy and procedure of the medication safety and advisory committee.
Corrective Actions Required: Ensure that the facility has a copy of the policy and procedures as established by the medication safety and advisory committee. The Manager stated they would contact the pharmacy to request this. Action/s Required by Licensing: Follow Up Inspection to be conducted.
