3040 - CCLR 53(1)(a)
Description: If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child’s parent or in the child’s record or care plan
Corrective Actions Required:
Corrective Actions Taken By Licensee: The Licensee stated that she would not issue medication to children that was not provided by the parent and not until a medication release form had been signed by the parent with the time and amount. This corrects the contravention.
Actions Required By Licensing: No further action required; this contravention has been addressed.

Observations: Licensee had medication provided by a parent but had not had the parent complete a medication administration form.
3070 - CCLR 56(1)(f)
Description: A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

Corrective Actions Taken By Licensee: Licensee provided Licensing with a copy of the facility minor accident and injury log.

3070 - CCLR 56(1)(f)
Description: A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Corrective Actions Required: Ensure that any minor injury to a child along with parent notification is noted in a log of minor accidents.

Observations: In discussion with the Licensee it was stated that a child had incurred a minor injury but that the event had not been documented nor the parent notified.

Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

Corrective Actions Taken By Licensee: Licensing was provided with children's records that indicated that all required information was on the records.

Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

Observations: A review of the four children's files indicated that the files did not contain the medical services plan numbers for any children.

Description: A licensee must keep, for each child, a record showing the following information:(i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency
Corrective Actions Required: Ensure that all children in care have a photo.

Observations: A review of the children's files indicated that one child had been missing a photo.
