Observations: Entanglement test device detected an entanglement on the right side of the slide of the composite climber. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. The Licensee Contact stated the slide would not be used until it was repaired. Full compliance will not be achieved until the plan has been implemented and completed.
